It must name three generations by relationship — the candidate's parent, that parent's own parent, and that grandparent's own parent — each with village or town, post office, police station and district, certifying all three were Permanent Residents of Assam. The fixed cut-off date for establishing that status is 1 January 2006. It must be signed and sealed by the District Commissioner or an authorised officer of the concerned district; without that signature, it will not be accepted.
