NEET Counselling for
Northeast India Students
The region has plenty of NEET-qualified students and very few seats to put them in. What follows is what each of the eight states actually offers, and where a Northeast candidate's realistic options sit once state quota is accounted for.
The structural problem. Assam, the largest state in the region, has no private medical college. The Northeast as a whole has almost none. So a student who misses their state government quota and does not clear All India Quota has to leave the region entirely — usually for Karnataka, Maharashtra or Uttar Pradesh at private fees. The two exceptions worth knowing are NEIGRIHMS Shillong, a central institute with reserved Northeast seats at government-level fees, and PA Sangma International Medical College in Ri-Bhoi, whose Management Quota has no domicile bar and sits an hour from Guwahati.

Distance Has a Price,
and It Is Not on the Fee Sheet
When a Northeast student takes a private seat in Karnataka or Maharashtra, the conversation at home is almost always about tuition. It should also be about the four flights a year, the guardian who cannot get there in a week when something goes wrong, and five and a half years of a student being the only person from their state in the batch. Some students handle that easily. Some do not, and the ones who do not tend to discover it in the second year, when changing course costs a great deal more.
This is not an argument against leaving the region. It is an argument for pricing the whole decision rather than the tuition line. Where a family is choosing between a distant private seat and a nearer one at similar cost, proximity is worth real money.
One warning specific to this region. Northeast families are heavily targeted by overseas MBBS agents, because the local seat shortage is well known. Before signing anything for a foreign medical degree, check the current FMGE pass rate for that specific university — not the country, the university — and read our page on what MBBS abroad actually involves. A degree that does not get you through the screening test is not a degree you can practise on.
One Trip In,
the Rest Over WhatsApp
Most of our students are not from Guwahati. Counselling registration, choice list building and deadline tracking work perfectly well over phone and WhatsApp, and during the season that is how most of the day runs.
What is worth travelling for is the first sitting, with a parent, and with the original documents on the table. Domicile and category problems are much faster to catch in person, and they are the single most common reason a Northeast student loses an otherwise valid allotment. If you can make one trip to GS Road, make it that one.

All eight have government medical colleges, but the counselling structures differ sharply. Assam runs DME Assam counselling across its government colleges. Meghalaya runs counselling through MSCAME, now including PA Sangma International Medical College. Manipur, Nagaland, Mizoram, Tripura, Arunachal Pradesh and Sikkim each handle state quota differently, and several have very few or no seats of their own, relying instead on central pool and reserved seats at institutions like NEIGRIHMS and RIMS Imphal. The right route depends on which state issued your domicile certificate, not which state you live in now.
Very few, and that is the defining fact of the region. Assam has none. The main private options within the Northeast are PA Sangma International Medical College at Baridua in Ri-Bhoi, Meghalaya, and Zoram Medical College in Mizoram for its NRI seats. PIMC is the significant one for most families here because its 64 Management Quota seats carry no domicile condition and it sits about an hour from Guwahati. Everything else means leaving the region.
The North Eastern Indira Gandhi Regional Institute of Health and Medical Sciences at Shillong is a central institute with 50 MBBS seats, of which a substantial share is reserved for candidates from the eight Northeast states. Its first-year fee is a fraction of any private college, and it carries a service bond. For a Northeast student with a mid-range rank it is often the single best value seat available, and it is consistently underapplied for relative to that.
That depends on the specific wording of the state rule, and it has changed. Assam amended its state quota eligibility criteria by Cabinet decision in October 2025, and several other states tie eligibility to where you did Class 10 and 12 rather than to a domicile certificate alone. Gujarat, Tamil Nadu and Telangana in particular test schooling location. Bring both your domicile certificate and your school records to a sitting — this is not a question that can be answered safely over a phone call.
Not as such. The 15% All India Quota through MCC is open to everyone on NEET rank, with the usual category reservations. Where Northeast candidates do have specific routes is in central pool seats, NEIGRIHMS and RIMS reservations, and state-specific arrangements that vary by year. Those are worth checking individually rather than assuming, because the seat counts are small and the notifications are easy to miss.
Further than families expect. In a typical year our students end up across Karnataka, Maharashtra, Tamil Nadu, Uttar Pradesh, Odisha and West Bengal, with a smaller number staying within the region at NEIGRIHMS, PIMC or a state government college. The practical point is that a college two thousand kilometres away has real costs beyond tuition — flights home, a guardian who cannot visit during an illness, and five and a half years of it. That belongs in the decision from the start, not as an afterthought.
